Step-by-Step Implementation Process
Step 1
Identify the key contact staff member(s) from your practice that will:
- Be the key contact for the TN Heart Health Network Practice Facilitator and the Evaluation Team
- Establish or modify the pharmacist partnership
- Implement the intervention outreach & scheduling
- Implement the billing codes for key pharmacist counseling services
Step 2
Meet with your TN Heart Health Network Practice Facilitator to fill out the Function & Form Matrix and the Memo of Agreement (MoA). These documents will walk you through the selection of options that will tailor the intervention to your needs and interests.
Step 3
Identify pharmacist partner:
- If using existing staff, re-evaluate/modify and designate network-related patient counseling hours
- If new staff, post your job description, hire, and provide clinic orientation
- If new external partnership, work with TN Heart Health Network Practice Facilitator to recruit pharmacist partner, execute Collaborative Practice Agreement, and provide orientation
Step 4
Register your pharmacist partner for Motivational Interviewing (MI) Certification Program by filling out the form here (please fill out a form for each participant)
Step 5
Meet with TN Heart Health Network Evaluation Team and key practice providers to:
- Discuss and develop plan for collection of metrics
- Discuss and develop plan to identify high risk patients that would benefit from pharmacist counseling services
Step 6
Train pharmacist and office staff to:
- Use CPT billing codes for selected pharmacist counseling services
Step 7
Create process (policies and procedures) for:
- Telephone outreach to high-risk patients
- Pharmacist referrals/warm hand-offs
- Documentation plan
Step 8
Introduce intervention and pharmacist partner to providers using PowerPoint Presentation for Overview of Pharmacist-Physician Collaborative Intervention
Step 9
Begin the intervention:
- Telephonic outreach
- Scheduling
- Pharmacist counseling services
- Documentation and collection of metrics
Step 10
Provide ongoing support for pharmacist partner:
- Supervisor for pharmacist partnership meets with pharmacist on regular basis to check on processes and procedures
- Pharmacist-Physician Collaborative Lead on TN Heart Health Network Team mentors pharmacist partners on both one-on-one and group conference calls
