Tennessee’s primary care and health system response to the pandemic of obesity- and smoking-associated CVD has been insufficient to meet its challenges. There is an insufficient pipeline of primary care providers to participate in traditional face-to-face visits which makes team-based care approaches like telehealth and lay health coaches to outreach and engage patients for population health efforts critical. The TN-HHN aims to assist PCPs gain more knowledge and resources to identify and implement proven financially sustainable population health approaches, especially because in Tennessee 95 of our counties have insufficient primary care.
