
The Pharmacist-Physician Collaboration will explore the benefits, operational aspects, financial sustainability, quality metric impacts and experiences of including pharmacists in primary care models. Pharmacists have been an integral part of the TN-HHN and helping to meet the primary care shortage in the state of Tennessee as well as helping patient’s to meet their CVD goals by addressing medication adherence and quality.
