Step-by-Step Implementation Process
Here is your step-by-step process for implementing the Heart Health Messages Intervention. If you have questions or need support along the way, feel free to contact your TN Heart Health Network Practice Facilitator.
Step 1
Identify the key contact staff from your practice who will oversee the intervention and serve as the key contact for the TN Heart Health Network Practice Facilitator.
Step 2
Meet with your TN Heart Health Network Practice Facilitator to fill out the Function & Form Matrix and the Memo of Agreement (MoA). These documents will walk you through the selection of options that will tailor the intervention to your needs and interests.
Step 3
Ensure that your patient data has been sent via secure transfer to the Tennessee Population Health Data Network (TN-POPnet). If you have questions or need assistance with this process, please contact your practice facilitator and they will connect you with our TN-POPnet staff.
Step 4
Introduce Heart Health Messages Intervention to clinic staff using the PowerPoint presentation Overview of Heart Health Messaging Intervention. This ensures that they are familiar with the program and can respond to patient inquiries.
Step 5
Post marketing materials around your clinic to familiarize patients with the program and assist with enrollment. Doctors and nurses should encourage patients to sign up. Be sure to put up posters and begin the conversations with patients before Step 6. (A template will be added soon.)
Step 6
Send a letter and/or an email invitation to your eligible patients (as identified by the TN-POPnet staff) from your practice we provide to participate in Heart Health Messages. (Sample invitation text will be added soon.)
Step 7
The University of Tennessee Health Science Center (UTHSC) will send a group of eligible patients two initial text messages, one inviting them to join the Heart Health Messages program and another with a survey link to their personalized survey consent form.

AFTER COMPLETING THE SURVEY CONSENT FORM (AVAILABLE HERE) YOUR PATIENT GETS THIS MESSAGE:
AT THIS POINT YOUR PATIENT RECEIVES A MESSAGE WITH A LINK TO A SHORT ENROLLMENT SURVEY TO CHOOSE THE FREQUENCY AND TYPES OF MESSAGES THEY WOULD LIKE TO RECEIVE.
AFTER COMPLETING THIS SHORT SURVEY, THEY RECEIVE THIS MESSAGE:
YOUR PATIENTS WILL NOW RECEIVE REGULAR TEXT MESSAGES FROM YOU IN ACCORDANCE WITH THE FREQUENCY AND TYPES OF MESSAGES THEY HAVE CHOSEN.
Step 8
Meet with the TN Heart Health Network support staff to develop and implement a sustainability plan for delivering Heart Health Messages to your patients after the initial six months of the program.
